"Fluid" shift on chest radiography: Bochdalek hernia.

چکیده
CMAJ © 2010 Canadian Medical Association or its licensors 1 A36-year-old healthy man presented with a suspicious pleural effusion on the left side that had been found on a pre-employment chest radiograph. He denied having any respiratory symptoms, and there was no history of trauma. On examination, he had decreased breath sounds and tactile fremitus on the left side. A posteroanterior chest radiograph showed blunting of the left costophrenic angle (Figure 1A), with more than 1 cm of shifting opacity seen on the lateral decubitus radiograph, consistent with free fluid (Figure 1B). Diagnostic thoracentesis failed to produce fluid. A computed tomography scan of the chest showed a prominent peritoneal fat herniation through a left posterolateral diaphragmatic defect, with no evidence of pleural fluid collection (Figure 2). The patient refused further management and has remained symptom free. A Bochdalek hernia, resulting from inadequate closure of the posterolateral pleuroperitoneal membrane, is the most common congenital diaphragmatic hernia, with an incidence of 1:2000 to 1:5000 live births. Congenital diaphragmatic hernias are usually found in neonates (approximately 10% of all reported cases occur in adults) and are associated with cardiac malformations and neural tube defects in up to 57% of patients. Chromosomal abnormalities, including trisomy 18 or trisomy 21, have been described in 10% to 20% of all cases. Defects occur more frequently on the left side (78%) than on the right side of the diaphragm, and abdominal contents, including stomach, bowel loops, liver, spleen or fat tissues, can be displaced into the thoracic cavity. Peritoneal fat, especially omental fat, tends to be mobile and can look like fluid on lateral decubitus chest radiographs. Clinical images

Adult Bochdalek hernia with bowel incarceration.
Bochdalek hernias are rare in adults. We report 2 cases of Bochdalek hernia with bowel obstruction. The first case was a 74-year-old male patient who suffered from abdominal pain and chest tightness for 1 day. Chest radiography indicated a mass-like lesion above the left diaphragm. The pain could not be relieved by nasogastric tube decompression for 12 hours. We arranged computed tomography, wh...
متن کاملLaparoscopic Repair of Bochdalek Hernia Into Contralateral Thoracic Cavity
Introduction: Bochdalek hernias in adults rarely become symptomatic. However, once the abdominal organs become incarcerated in Bochdalek hernia, surgery is required to reduce them from the thoracic cavity. Case Description: We present the case of a 71-year-old woman with continuous chest discomfort and spontaneous epigastralgia. Computed tomography demonstrated that the colon and omentum were h...
متن کاملAbdominal compartment syndrome post - late Bochdalek hernia repair : a case report
The aim of this case report is to discuss the rare postoperative complication of abdominal compartment syndrome in a 19-year-old Caucasian Greek male that was electively operated on for a congenital diaphragmatic hernia. The hernia was completely asymptomatic and was found in chest radiography for employment reasons. Abdominal compartment syndrome is related in most reports with trauma and abdo...
متن کاملSilent Tachypnoea in a Neonate: A Rare Presentation of Right Side Bochdalek Hernia with Intrathoracic Kidney
Congenital diaphragmatic hernia (CDH) is a rare condition. The reported incidence of intrathoracic renal ectopia due to CDH is also rare. A right-sided thoracic kidney is much less common due to the location of the liver. Isolated intrathoracic kidney is usually asymptomatic and diagnosed incidentally on chest imaging. The authors report on a 21days old female infant with late-presenting right ...
متن کاملBochdalek hernia presenting in adult life: report of an unusual case and review of the literature
Below, we present the case of a middle-aged woman with a symptomatic congenital posterior diaphragmatic (Bochdalek) hernia that remained undiagnosed for years. During the last attack, when she was referred to our institution, she had a large thoracic herniation of several abdominal viscera including the stomach, which was also rotated along its transverse axis (gastric volvulus). After a chest ...
